Proxy User Registration for Interlibrary Loan

This registration form is meant only for use by registered proxies.

If you are not a registered proxy, please return to the Interlibrary Loan logon screen and log in to your Interlibrary Loan account using your NetID and password.

In order to request Interlibrary Loan materials on behalf of faculty, staff or students registered with Disability Services, the person who wishes you to be their proxy will need to fill out the following form to obtain a proxy card for you: Proxy Card Application

Username and Password

Once you have your proxy card, you can register for your Interlibrary Loan Proxy Account. Your proxy card barcode will be your Username and you will set your own Password. You can find your proxy barcode on the back of your proxy card. It begins with the digits 212066...

Note that you will also have a personal Interlibrary Loan account that you will be able to log in to using your NetID and password.
